Responsibilities include:
Providing direct patient care for assigned patients.
Evaluating clinical needs; developing, implementing, and revising the patient's care plan; documenting accordingly.
Utilizing clinical grouping guidelines for interdisciplinary utilization and data entry of initial visit frequency.
Ensuring OASIS assessments accurately reflect documentation.
Updating the Clinical Manager on referrals to community resources.
Participating in interdisciplinary case conferences and evaluating care effectiveness.
Supervising Home Health Aide care plans every 14 days with documentation.
Notifying care providers and managers of significant patient condition changes.
Completing admission and discharge paperwork promptly and accurately.
Documenting skilled care clearly and concisely, including teaching, direct care, and patient/caregiver responses.
Obtaining and clarifying physician orders for all care.
Planning and coordinating discharges with the team, informing patients and physicians, and completing discharge documentation.
Performing all other duties as assigned.
Passing the VNA pre-hire medication test.
Holding a current RN license in Massachusetts and relevant credentials.
Having at least one year of nursing experience, or six months with a BSN.
Maintaining CPR certification.
Preferred: Home care and IV experience, computer skills, and a BSN.
Position Details Responsible for comprehensive nursing assessments upon patient admission, considering physiological, psychological, environmental factors, and health history, to ensure safe and quality care. Collaborates with the clinical team to integrate assessment data and identify patient problems.
Schedule: 37.5 hours/week, 8 am - 4 pm, rotating weekends and holidays. Sign-on bonus available.
