Overview
Physical Therapist – Sign-On Bonus Eligible. Req#: R36076. Baystate Health, western Massachusetts’ leading academic health system with a Level 1 trauma center for complex spine and brain injuries, seeks a full-time Physical Therapist for the Greenfield, MA outpatient setting. Responsibilities
Evaluates all assigned patients, develops plan of treatment, provides assessment and assists in discharge planning as appropriate for each discipline. Evaluate within 24 hours using established procedures; plan goals and treatment; assist patient/family with equipment needs and discharge planning with rehab team. Apply treatment per established plan; modify as needed; train patients and families on carryover and equipment; maintain safe environment and demonstrate empathy. Complete required documentation for initial consult, progress notes, and discharge reports within established timeframes; document patient history, results, prognosis, and recommendations. Provide information and consultation to patients, families, and other health care personnel; collaborate with MDs, nursing, and ancillary services as indicated; document counseling when appropriate. Complete department operational procedures (scheduling, billing, equipment usage, time-off requests, caseload coverage) and submit charges within 24 hours; support staff during shortages as needed. Assist with on-the-job orientation and training for student interns; contribute to intern evaluation and rating processes. Demonstrate clinical knowledge in evaluation, treatment, and counseling; establish professional development objectives; meet productivity expectations as determined by supervision. Attend in-service trainings and seminars as directed; may present in-service trainings within guidelines. Qualifications
Bachelor’s degree in Physical Therapy (Doctorate preferred) Massachusetts Physical Therapist Licensure (required) New grads welcomed to apply American Heart Association BLS CPR required Location & Schedule
