Ideal candidate would service both Marietta and McConnelsville areas. Community-based position. SUMMARY:
PATH Behavioral Health, an expanding force in behavioral healthcare, seeks outstanding individuals to join our dynamic team. We are dedicated to redefining mental health care by offering clients a fully integrated system of care with zero barrier access. Our mission is to provide compassionate, high-quality mental health services, especially to underserved populations. We are extending our reach across Ohio and invite a Case Manager to join our collaborative care team, working alongside nurses, nurse practitioners, case managers, and therapists within our one-stop agency. Join us in reshaping mental health services. KEY RESPONSIBILITIES Act as a liaison between care team members and clients' families. Arrange family visits, coordinate transportation, and meet clients' needs. Maintain clear communication between families, referral sources, and PATH Behavioral Healthcare. Engage in professional case management to support clients' mental and physical health. Assess clients' needs and develop personalized care plans. Collaborate with care team members to ensure seamless care delivery. Monitor clients' progress and adjust care plans as needed. Advocate for clients to access resources and overcome obstacles. QUALIFICATIONS Education:
High School Diploma or GED required; Bachelor’s degree preferred. Experience:
Minimum of 1 year of case management experience. Physical Effort:
Ability to work under stress and irregular hours. Communication Skills:
Excellent verbal and written communication, with strong grammar skills. Interpersonal Skills:
Friendly, compassionate, and professional interaction with colleagues, clients, and families. Technical Skills:
Competency in Electronic Health Records (EHR), telehealth, and other relevant software. Good dexterity for handling equipment and tasks. Background:
Must pass all federal and state background checks. Educational and work history will be verified.
