Complete a health assessment for clients upon entry to CSN Track annual health assessments Initiate and direct implementation of Nursing Care Plan Provide professional nursing care Assist and Coordinate medication monitoring Serves as an interdisciplinary treatment team member Attends case conferences, required meetings, seminars and in-service trainings Responds to request for specialized reports Qualifications
