Job Summary To work within Gandara Residential Services team model, establishing open communication, support, and accountability among workers and consumers. The case manager is accountable to the Program Director/Clinical Supervisor, who is responsible for providing supervision and training. The Case Manager staff actively promotes the philosophy of Gandara Center and maintains a high level of commitment to the program.
Duties and Responsibilities
Share all program expectations and responsibilities with RSII and RSI
Provide advocacy/case management services
Develop strong, supportive relationships with clients and provide for client care and safety
Complete all clinical documentation and casework for assigned residents: adult comprehensive assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, and transition/discharge/aftercare planning
Maintain updated clinical notes and reporting in accordance with established record keeping procedures
Facilitate daily evidence-based psychoeducation groups and other recovery-based groups
Provide weekly individual case management sessions with assigned residents
Administer and complete the EIM/ESM forms and adhere to funding sources requirements
Maintain direct supportive contact with residents to continually assess their needs, provide information, discuss problems, set limits and encourage growth
Be capable of assuming enhanced responsibilities for planning and overseeing on and off unit activities
Develop and maintain working relationships with other social service agencies in the community
Notify supervisory staff immediately in the event of a client crisis, suspected illegal activity, or any incident in the program
Participate in all case consultation meetings, staff meetings, and case conferences
Administer drug/alcohol screens to residents when clinically appropriate
Provide orientation of services to prospective clients, family members and community groups
Utilize de-escalation techniques to reduce, prevent, or resolve escalated resident conflicts
Arrange patient transportation to outside appointments when public transportation is not available or recommended
Participate in training programs (external and internal) and regularly research materials related to chemical abuse, mental illness and treatment approaches
Acquire and maintain current certifications mandated by the Agency and funding sources
Adhere to program and agency policies and procedures
Monitor resident medication and complete hourly checks as needed (may be required)
Perform other duties as assigned by the Program Director/Clinical Supervisor
Administrative Responsibilities / Record Keeping
Run shifts in the absence of a supervisor
On-call duties as needed
Attend scheduled staff meetings
Screen incoming calls and forward information through proper channels
Maintain client records in accordance with agency guidelines and timetables
Attend scheduled supervisory meetings with Program Director/Clinical Supervisor
Maintain assigned financial record keeping and submit required reports accurately and timely
Attend trainings as required by funding source and the Agency
Complete time sheets and submit them accurately and in a timely manner
Minimum Qualifications
Associates Degree + 2 years experience
Bachelor’s degree + 1-2 years’ experience preferred
Ability to work with population in recovery
Ability to structure and organize job responsibilities with minimal supervision
Ability to master First Aid, CPR, Defensive Driving and Fire Safety
Good verbal and written communication skills
Sound judgment and ability to provide goal-oriented services
Works well as a team member
Bilingual (Spanish/English)
