Client Services Responsibilities
Share all program expectations and responsibilities with RSII and RSI Provide advocacy/case management services Develop strong, supportive relationships with clients and provide for client care and safety Complete all clinical documentation and casework for assigned residents (adult comprehensive assessment, individual action plans, group progress notes, weekly progress notes, discharge summary, transition/discharge/aftercare planning) Maintain updated clinical notes and reporting in accordance with established record keeping procedures Facilitate daily evidence-based psychoeducation groups and other recovery-based groups Provide weekly individual case management sessions with assigned residents Administer and complete the EIM/ESM forms and adhere to funding sources requirements Maintain direct supportive contact with residents to assess needs, provide information, discuss problems, set limits and encourage growth Capable of assuming enhanced responsibilities for planning and overseeing on and off unit activities Develop and maintain working relationships with other social service agencies in the community Notify supervisory staff immediately in case of a client crisis, suspected illegal activity or any incident in the program Participate in all case consultation meetings, staff meetings and case conferences Administer drug/alcohol screens to residents when clinically appropriate Provide orientation of services to prospective clients, family members and community interest groups Utilize de-escalation techniques to reduce or resolve escalated conflicts Arrange transportation to outside appointments when needed Participate in training programs and regularly research materials related to chemical abuse, mental illness and treatment approaches Acquire and maintain current certifications mandated by the Agency and funding sources Adhere to program and agency policies and procedures May monitor resident medication and perform hourly checks as needed Perform other duties as assigned by the Program Director/Clinical Supervisor Administrative Responsibilities/Record Keeping
Run shifts in absence of a supervisor On-call duties Attend scheduled staff meetings Screen incoming calls and forward information through proper channels Maintain client records in accordance with agency guidelines Attend scheduled supervisory meetings Maintain assigned financial record keeping and submit required reports accurately and timely Attend trainings as required by funding source and the Agency Complete time sheets and submit them accurately and timely Minimum Qualifications
Associates Degree or CADC + 2 years experience Bachelor\'s degree or LADC + 1-2 years’ experience preferred Demonstrated strong communication and interpersonal skills, including conflict resolution Experience providing substance abuse services Demonstrated organizational skills, able to multi-task Demonstrated ability to lead through example Must have valid driver\'s license and meet Agency requirements for driving history Must have availability of an automobile to transport clients or perform other supportive services as necessary Referrals increase your chances of interviewing at Gandara Center by 2x Get notified about new Case Manager jobs in Ludlow, MA.
