Position Summary

The case manager will provide support and service linkages to residents within the program. Coordinate services and ensure quality care to the residents. Essential Accountabilities

Routinely visit apartments and move freely through the apartments to observe and evaluate level of functioning of residents. Provide direct care services to residents, including planning and implementation of goals and training to develop and enhance activities of daily living skills, including but not limited to: budgeting; grooming; nutritional planning; use of public transportation/travel training; use of community resources/community integration; shopping/menu planning; housekeeping; socialization skills; physical health maintenance; assertiveness/self-advocacy training. Maintain all necessary written documentation including functional assessments (initial and updated), medication updates, progress notes and reports, and annual/quarterly service plans. Screen candidates for residential placement and participate in meetings/trainings at various sites; follow different routes of travel to worksite; attend resident meetings and conduct 30-day follow-ups on discharged residents. Assist residents in planning and coordination of recreational/social activities, including escorting residents and participating in moderate physical activities as appropriate. Attend all staff meetings and ensure resident meetings occur on a timely basis. Open and follow up for new apartments; maintain, upkeep, and ensure safety of assigned apartments. Report and document all incidents to the immediate supervisor and/or the Director of Behavioral Health within 24 hours; maintain resident files monthly. Provide crisis intervention as needed. Monitor environmental plant issues and report issues to the supervisor or Director of Behavioral Health. Participate in activities to support other staff members in their absence or during shortages. Represent the agency at meetings and trainings as required. Qualifications

Associate’s Degree in Human Services; or a High School diploma with two (2) years experience in the Human Services field. New York State driver’s license and access to a vehicle required. Benefits and Perks
