Duties Provide professional social work services to individuals with mental illness in a recovery‑oriented environment. Duties include:
Providing crisis intervention, service linkages, and discharge planning
Developing treatment plans with individuals and the treatment team; monitoring and evaluating progress; modifying plans as needed
Determining discharge needs and appropriate community resources
Establishing and maintaining relationships with community providers to ensure continuity of care
Minimum Qualifications Candidates may be considered through the Hiring for Emergency Limited Placement – Statewide (NY HELPS) program if licensed and registered as an LMSW in New York State, or transfer to a competitive class position if they have one year of permanent competitive or 55‑b/55‑c service in a title eligible under Section 70.1 of the Civil Service Law.

The position is located at Central New York Psychiatric Center's Five Points Residential Mental Health Unit on the grounds of Five Points Correctional Facility, Seneca County.

If the license and/or registration is not maintained once appointed, action will be taken to remove the individual from the position.
To maintain employment, the individual cannot be listed as an excluded entity on any Federal or State Medicaid and Medicare exclusion lists. If appointed and subsequently listed on any such list, termination may occur.
Appointed individuals may be required to become an enrolled Medicare provider and provide a National Provider Identifier (NPI). Failure to bill through Medicare and Medicaid may result in termination.
