Evaluate and document evaluations of referred residents in accordance with facility policies and procedures. Provide appropriate treatment to help residents maintain or attain the highest level of function possible within their physical abilities. Recommend and/or provide appropriate treatment, preventative measures and adaptive equipment to prevent decline in residents’ function. Maintain timely and accurate resident reports as required by federal, state and local regulatory agencies and third-party payers. Observe infection control procedures. Be responsible for the safety of residents under supervision. Assume duties related to completion of Minimum Data Set (MDS) assessments as assigned. Complete documentation related to evaluations, treatment plans, MDS assessments, resident care plans and progress notes in a timely manner. Participate in resident care conferences, interdisciplinary care conferences and rehabilitation conferences as scheduled. Participate in implementation of programs to improve resident care related to Quality Indicators. Assume Quality Assurance duties as assigned. Collaborate with residents, residents\' representatives, facility staff, physicians, consultants and ancillary service providers. Observe all facility policies and procedures, safety policies and the employees’ handbook guidelines. Follow Residents\' Rights policies and Corporate Compliance Program Code of Conduct and policies. Attend Corporate Compliance Program involvement as required and comply with facility policies against discrimination and sexual harassment. Protect the confidentiality and security of all resident and facility information. Come to work in clean, neat attire and consistently present a professional appearance; be dependable and punctual. Accept assigned duties cooperatively and perform all duties effectively, timely and professionally. Support and respect other employees; perform other related duties as directed by supervision. All other duties as assigned. Qualifications

Communication/Language Skills — Excellent verbal and written communication and organizational skills. Ability to read, analyze, and interpret common medical information. Ability to communicate effectively with residents, families and other healthcare professionals. Ability to present information to supervisor. Reasoning Ability — Ability to define problems, collect data, establish facts, and draw conclusions. Computer Skills — Familiar with Word and Excel. Doctorate of Physical Education Active Physical Therapist Licensure Seniority level
