Bedrock Healthcare . The MDS Coordinator is responsible for overseeing the Minimum Data Set (MDS) assessment and care planning process in accordance with state and federal regulations. This role ensures accurate and timely completion of assessments that impact resident care, reimbursement (PDPM), and quality measures. The MDS Coordinator works closely with the interdisciplinary team (IDT) to maintain compliance and promote optimal resident outcomes. Responsibilities
Coordinate and complete timely MDS assessments (initial, quarterly, annual, and significant change). Ensure all MDS data is accurate, complete, and compliant with CMS regulations. Submit MDS assessments through the CMS QIES ASAP system. Collaborate with the Interdisciplinary Team (IDT) to develop and update individualized resident care plans. Lead or participate in resident care plan meetings. Monitor PDPM classifications to ensure accurate reimbursement and documentation. Train and educate nursing and clinical staff on documentation best practices related to MDS and care planning. Conduct audits of clinical documentation to support MDS coding and identify discrepancies. Monitor Quality Measures (QMs) and assist in implementing quality improvement initiatives. Maintain current knowledge of MDS 3.0 guidelines, state and federal regulations, and facility policies. Prepare for and participate in state and federal surveys and respond to deficiencies. Maintain confidentiality of all resident records and information. Other Duties
Assist the immediate supervisor with various administrative tasks to support department operations as directed; may also be assigned special projects. Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances. Meet ongoing requirements to maintain current certified nursing assistant certifications. Interacts with residents, families, visitors, facility and corporate staff. Carries out other tasks as requested in situations where hands-on intervention may be required. May be required to work rotating shifts, holidays, weekends, additional hours, and in different positions as the facility’s needs require. Must practice good personal hygiene and follow the dress code requirements. Must provide annual verification of a negative TB skin test as required. Support an atmosphere which promotes a homelike environment. Supports, cooperates with, and implements specific procedures and programs for safety, confidentiality, quality assurance and regulatory compliance. Promotes positive public relations with residents, family members and guests. Completes requirements for in-service training and attendance, uniform and dress codes including personal hygiene, and other work duties as assigned. Qualifications
Required:
Valid Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of employment. Experience in long-term care or skilled nursing setting. Knowledge of MDS 3.0, RAI process, and PDPM. Proficiency in electronic health record (EHR) systems and MDS software (e.g., PointClickCare, MatrixCare). Strong organizational and time management skills. Attention to detail and ability to meet strict deadlines. Preferred:
RAC-CT (Resident Assessment Coordinator – Certified) credential through AANAC/AHCA. At least 1 year of MDS coordination experience. Familiarity with quality initiatives such as QAPI and Five-Star Quality Rating. Employment details
