Job Overview:
The Pharmacist I optimizes patient safety through medication therapy management, accurately compounding sterile and non-sterile products, providing drug information to patients and staff, and providing direct oversight of technician activity while ensuring compliance with regulatory requirements and hospital policies.
Qualifications:
Required:
Completion of course at an accredited School of Pharmacy, with a degree of Bachelor of Science in Pharmacy or PharmD
Experience compounding of sterile and non-sterile productions utilizing USP 795, 797, 800
Preferred:
Board Certification
Certifications and Licenses Required:
Current NJ State Pharmacy License with the NJ Division of Consumer Affairs
Scheduling Requirements:
Shift working, mostly coverage for evenings and weekends
Per Diem (PD)
Essential Functions:
Perform clinical review of all physician orders and make necessary interventions prior to dispensing medications
Oversee all aspects of medication management, including medication distribution, medication procurement and storage, sterile product preparation and medication therapy monitoring
Reviewphysician orders to assure accuracy and appropriateness of treatment, intervenewith prescriber and documentinterventions as required
Followthrough and documentall orders requiring clarification and/or alteration in a responsible and accountable manner in accordance with the highest professional standards, formulary compliance and hospital policy
Workcollaboratively with pharmacy technicians and other health care professionals
Verifyand monitormedication orders in accordance with established policies and procedures in order to meet regulatory requirements and document patient care interventions
Followall procedures involved in the appropriate documentation of all medication errors and adverse drug reactions
Followall procedures involved in the appropriate clarification of patient orders using sound clinical judgement, evidenced-based medicine and established clinical initiatives
Workin conjunction with department personnel in order to keep the department neat, organized, and regulatory sound
Superviseand participatein the maintenance of the compounding room environment, followall USP, NJ State Board of Pharmacy, and medication management regulations
Completethe daily operations of sterile compounding as stipulated by the department work flow
Completedepartment clinical initiatives such as IV to PO conversions, renal dose adjustments, clarification of orders, and following medication shortage strategies
Performall other duties as requested by Management
