Overview Physical Therapist - Outpatient role at Huggins Hospital. Under the general supervision of the Director of Rehabilitation and Diagnostic Services, administers Physical Therapy services including evaluations, establishment/modification of treatment plans, provision/supervision of treatment, and documentation for inpatient and outpatient settings in accordance with departmental and facility policies. Aims to restore patient function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy.

Responsibilities

Clinical Evaluation: validate provider prescription prior to evaluation and treatment; provide evaluation and treatment according to the provider’s order; contact provider for clarification as needed.

Perform evaluations and reevaluations according to departmental policies.

Evaluate effects of treatments and the fit of prosthetic/orthotic devices by observing progress; recommend adjustments as needed.

Use objective measures in evaluations; write measurable/functional goals based on objective findings.

Develop treatment plans with patient input and in accordance with provider orders.

Treatment and Discharge Plan: address patient needs (age, physical, psychological, learning); encourage patient participation in care plan and goals.

Apply therapeutic exercises, functional training, manual techniques, assistive devices, and physical agents to treat patients.

Educate patients on ongoing goals as they are achieved or modified; inform about discharge timing.

Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contribute to patient care conferences.

Ensure continuation of therapy after discharge by designing home exercise programs and instructing patients, families, and caregivers; recommend or provide assistive equipment; suggest outpatient or home options as appropriate.

Quality and Customer Service: greet patients by name and on time; ensure thorough documentation in line with departmental policies and insurance requirements; manage charges accurately; participate in mandatory competencies and monthly chart audits; maintain professional knowledge through education and professional activities; contribute to staff development through in-service training.

Other: comply with laws and professional standards; stay current with insurance requirements; teach/lead PT students and educate other health providers about Physical Therapy; supervise PTA and maintain ongoing communication; collaborate with other health professionals; travel to off-site approved locations as needed.

Organizational Expectations: attendance, HIPAA compliance, internal communication, interpersonal relationships, and safety responsibilities as part of daily practice.

Employee Engagement: participate in Service Excellence initiatives and trainings; attend department meetings and review minutes; complete required annual education on time.

Knowledge, Skills and Abilities

Ability to perform physically demanding tasks with occasional lifting up to 50 pounds; capable of stooping, finger use, and fine motor skills.

Ability to manage mental stress effectively and communicate clearly in speech and writing.

Proficient with documentation, computers, and business machines.

Qualifications

Education: Bachelor's and/or Master’s and/or Doctorate in Physical Therapy from an APTA-accredited institution.

Experience: 3–5 years of hospital and outpatient setting experience.

Licensure/Certification: BLS required; licensed in NH by the NH State Board of Allied Health.
