Overview CHARACTERISTICS OF THE WORK: Under supervision, performs nursing work. Responsible for assisting providers and delivering nursing care in a primary care center under the auspices of the Family Health Centers (FHC).

Responsibilities

Documents patient history and assesses chief complaints.

Reviews charts and makes entries.

Prepares patient and room for appropriate examination procedures.

Assists providers with exams and procedures.

Administers medications, injections, immunizations, and venipunctures as ordered by physician.

Handles all abnormal and normal laboratory test results according to protocol.

Provides individual and group health education instruction and information.

Mentors and acts as a resource for health services students and departmental personnel.

Provides appropriate patient care based on age-appropriate growth and development principles.

Assists in triage, handles patient inquiries, schedules appointments, and assesses patient complaints via telephone and recommends appropriate action, according to prescribed protocol.

Performs nursing assessments and develops health care plans for patients.

Performs basic laboratory procedures.

Makes referrals to the Registered Nurse (RN), supervisor, physician, or Advanced Practice Registered Nurse (APRN) for situations requiring further intervention.

Cleans, stocks, and orders supplies for the examination rooms.

Cleans and wraps instruments for sterilization.

Travels between satellite clinics.

Adheres to standard precautions.

Performs related work as required.

Minimum requirements Possession of a current Licensed Practical Nurse (LPN) license issued by the Kentucky Board of Nursing OR possession of a work permit to practice professional nursing in the State of Kentucky by endorsement.

Physical requirements and working conditions

Moves and/or lifts more than 100 lbs. when positioning patients or when assisting them on and off the examination table.

Identifies medical record documentation, requests and/or releases.

Responds to patient and FHC needs via telephone, internet, fax, computer software and in person.

Special requirements

Must obtain CPR certification by the end of the probationary period and maintain certification throughout employment with FHC.

Must provide own transportation to work at other FHC locations as needed.

Must be available to work a varied schedule as needed.

Must wear personal protective equipment (PPE) as mandated by OSHA and/or FHC.

Must be vaccinated for Hepatitis B, have provided evidence of immunization, or have signed a declination form within 10 days of initial assignment.
