Job Summary The On Call Nurse provides direct patient care for patients with life limiting illnesses. The patient population can include newborns through geriatric patients. The On Call Nurse uses the Nursing Process to manage the Plan of Care based on nursing standards, policies and procedures. Manages patient care duties and delegates to team members to assure optimal patient outcomes. Maintains personal development to enhance nursing practice. Performs other duties as assigned. Primary Job Duties Schedule: Covering Monday-Thursday nights from 5pm-8am. Salaried role, plus differentials. Territory: Onslow/Pender/Duplin, backup to New Hanover county Candidate must be willing to cover entire area but will operate with a FT On Call counterpart. Expectations: Triage after hours calls; make visits. 0-12 visits per night. Normally less than 6 visits per night. Utilizes all aspects of the Nursing Process (assessment, planning, implementation, evaluation) to assess patients for eligibility for agency programs and to manage the Admission Plan of Care. Provides appropriate patient education to the patient/family according to the Plan of Care to facilitate comfort and support in life-limiting illness. Documents and communicates all required information in accordance with agency policies, accreditation, regulation, and legal requirements. RN delegates specific patient care duties to LPNs and NAs. LPNs work under the direct supervision of the RN. Continues personal development to enhance practice and profession. Organizes and performs work effectively and efficiently. Maintains and adjusts schedule to enhance program performance. Demonstrates positive interpersonal relations with the team. Ability to work independently while managing hospice patient care. Maintains and promotes customer satisfaction. Demonstrates the mission, vision, and values of Lower Cape Fear LifeCare daily. Job Specifications Education: Graduate of an approved school of nursing (AD or BSN). Licensure/Certifications: Current nursing license in North Carolina or South Carolina. Experience: Two years preferred; hospice, home care, or related oncology experience. Technical/Motor Skills: Hand/eye coordination; ability to use computer; clear communication with patients, families, physicians, and staff. Interpersonal Skills: Positive interaction with patients, families, physicians, and staff. Physical Requirements: Lift/move objects up to 45 pounds; ability to push/move patients. Mental Abilities: Assess conditions, formulate and implement Plan of Care; memory for patient information and care planning. Sensory Requirements: Visual and auditory abilities to assess and communicate; auscultate lung and bowel sounds. Hazards: Exposure to blood and body fluids, infections, needle punctures; possible safety risks during home visits; weather changes. Hours of Work: RN weekday call hours are 5:00pm - 8:00am, Monday-Friday mornings. Driving: May be required; reliable transportation; Safe Driver Policy.
