Physical Therapist - Apache Junction (PT) (Healthcare)
Evaluate patients within twenty-four (24) hours or one business day of physician referral. If unable to complete the evaluation in 24 hours, document the reason for the delay in the medical record. Develop an effective treatment plan and obtain approval from the referring physician. Treat patients according to the physician treatment plan.
Assist the nursing department with training Restorative Aides. Supervise Physical Therapy Assistants in direct patient care and related activities, following state practice acts. Communicate patient progress, problems, and plans with supervisors and other health team members.
Participate in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as needed. Conduct in-service training for staff. Record evaluations, treatment notes, progress notes, and recertification documentation in compliance with PT Board State Practice Act and payer requirements. Document treatment changes per policies.
Instruct patients' families and nursing staff in maintenance programs and caregiver training for discharge preparation. Participate in discharge planning. Recommend and facilitate the ordering of durable medical equipment such as braces, walkers, canes, and grab bars to promote patient independence.
Report equipment issues to ensure maintenance. Relate positively and effectively with patients, families, staff, and colleagues.
Supervisory Requirements:
Supervise PT assistants, aides, and students. Qualifications: Bachelor's degree in Physical Therapy; Master's or Doctorate preferred. Proficient in computer skills (e.g., Casamba, Point Click Care). Ability to read and comprehend technical procedures, policies, and manuals. Effective communication skills with managers, coworkers, and families. Ability to apply mathematical concepts and solve practical problems. Interpret instructions in various formats. Licensed as a Physical Therapist in the state; ongoing continuing education required. Physical Demands:
Frequent standing/walking, reaching, talking, hearing, and smelling; occasional sitting, climbing, kneeling, crouching; ability to exert significant force. Work Environment:
