Speech Therapist Home Health Per Diem

Southwoods Health is hiring a Per Diem Home Health Speech Therapist. Responsibilities Administers medically prescribed speech therapy treatment to patients suffering from injuries or disease to restore function, relieve pain and prevent disability. Reviews physician referral/orders, patient’s condition and medical records to determine speech therapy treatment needs. Develops and revises speech therapy treatment plans as appropriate based on evaluation and continued monitoring of patient strength, motor development, sensory perception, functional capacity, respiratory and circulatory function, records, and response to treatment. Confers with physician and other practitioners to obtain additional patient information, suggest revisions in treatment program and integrate speech therapy treatment with other aspects of patient’s health care. Determines referral needs and makes applicable referrals in a timely manner. Provides appropriate level of supervision for speech therapist students, other licensed personnel and unlicensed personnel according to state and federal requirements. Documents patient care services in accordance with regulatory requirements and within time expectation set by facility standards. Provides education to patients and patient families as appropriate, including information on post-op recovery processes, physician protocols, treatment plan, and home exercise programs. May plan and conduct in-services or training programs on speech therapy and related topics for medical staff, students and/or community groups. Demonstrates flexibility in work schedule to meet patient scheduling needs.
