Practice Nurse - Treatment Room

Kingswood Health Centre are looking to recruit a new Practice Nurse for our Treatment Room, following the retirement of one of our team.
This role is part of an established team of nurses, Trainee Nursing Associate and Healthcare Assistants who play an integral role in the service we offer to our patients.
The role will also support our PCN's Leg Club once a week based at Hanham Community Centre currently. This holistic service supports patients receiving treatment for lower leg wounds in a holistic way, with input from local VCSE services and community partners.
This post is for up to 37.5 hours per week - working pattern to be negotiated. Interviews will be held on Tuesday 25th November.
Main duties of the job
The Practice Nurse delivers nursing care to our practice population. This includes wound care, cervical sample taking, health promotion and lifestyle advice, vaccinations and immunisations, ear irrigation and travel advice and vaccinations. There is the opportunity in the future to upskill in supporting patients with chronic disease management (e.g. diabetes, asthma, COPD, hypertension etc.) About us
Kingswood Health Centre is a busy surgery operating in an urban environment with mixed demographics of working-age people, elderly patients, young families and pockets of deprivation. We pride ourselves on good access for our patients, high quality care and a supportive team. Owned and operated by 7 GP Partners and a Managing Partner, we also have a multidisciplinary team of GPs, Pharmacists, Physiotherapy, Social Prescribers and Care Coordinators. Our amazing nursing team consists of 5 trained practice nurses, 1 trainee nursing associate and 3 healthcare assistants, led by our Lead Nurse, Rachel. We are a caring, supportive team working to a very high standard of care. Assess, plan, develop, implement andevaluate programmes to promote health and well-being, and prevent adverseeffects on health and well-being Implement and evaluate individualtreatment plans for patients with a known long-term condition Identify, and manage as appropriate,treatment plans for patients at risk of developing a long-term condition Prioritise health problems andintervene appropriately to assist the patient in complex, urgent or emergencysituations, including initiation of effective emergency care Support patients to adopt healthpromotion strategies that encourage patients to live healthily, and applyprinciples of self-care Deliver opportunistic health promotionusing opportunities such as new-patient medicals Provide information and advice onprescribed or over-the-counter medication on medication regimens, side effectsand interactions Support patients to adopt healthpromotion strategies that promote patients to live healthily, and encourageprinciples of self-care Assess and care for patients presentingwith uncomplicated wounds Support and advise women requestinginformation relating to family planning needs Support and manage health needs ofwomen presenting for cervical cytology consultations Recognise, assess and refer patientspresenting with mental health needs in accordance with the National Framework(NSF) for Mental Health Implement and participate invaccination and immunisation programmes for both adults and children Advise, support and, where appropriate,administer vaccinations for patients travelling abroad Promote and deliver evidence-based carefor patients presenting with aural conditions Assist senior practitioners inproviding minor-surgery sessions Person Specification
Qualifications
Registered Nurse (with the UK Nursing & Midwifery Council) Diploma or further qualification in any area of chronic disease management (e.g. diabetes, hypertension, asthma, COPD etc.) Experience
Experience of working with vulnerable patients Experience of working as part of a multidisciplinary team Experience of wound care and management ECGs and other observations Vaccinations and immunisations experience Experience of delivering health promotion to patients Experience within a primary care setting (GP practice, district nursing etc.) Cervical sample taker Chronic disease management experience Experience using the EMIS Web patient record system Understanding of the Quality and Outcomes Framework Experience of travel medicine Experience with family planning Skills & Abilities
Able to work autonomously Understanding of infection prevention and control Ability to work under pressure Ability to work as part of a team Strong communication and interpersonal skills Reliable, personable and flexible Disclosure and Barring Service Check
