CAMBA offers more than 180 integrated programs in Education & Youth Development, Family Support, Job Training & Employment Support Services, Health, Housing, and Legal Services, serving a diverse cross section of New Yorkers. CAMBA operates 6 single adult shelters and 6 family shelters and one drop-in center. Each shelter offers clients a supportive, structured therapeutic, safe, and drug-free facility. Homeless individuals are provided with comprehensive services to stabilize their condition and transition to permanent and/or supported housing. Atlantic House Men’s Shelter is a 200-bed facility in Brooklyn, with many clients duall y diagnosed with mental illness and substance abuse. What The Case Manager Does

Under general supervision, formulate a plan and prepare clients for permanent housing placement. Engage individuals requiring social service assistance. Interview and evaluate clients, formulate Independent Living Plans and goals. Locate and refer to appropriate community resources for clients, including on-site medical and psychiatric services where applicable. Conduct initial intake, psychosocial evaluation, and assessment of clients’ needs and periodic re-assessments of independent living plans (short-term and long-term goals). Create and maintain client electronic and paper files. Assist clients in attaining their goals by identifying barriers and referring to appropriate services within and outside CAMBA. Advocate for clients to gain housing stability and encourage participation in the process toward permanent housing. Monitor progress through regular face-to-face contacts/sessions. Document and update client encounters via progress notes in DHS CARES Database and in paper files. Follow up with clients and referral organizations regarding contact and progress. Serve as liaison/advocate with outside organizations on education, healthcare, housing, and legal issues as needed. Schedule and escort clients to appointments with referral organizations as required. Assist clients in completing applications for benefits and entitlements as needed. Qualifications

Minimum Education/Experience: Bachelor’s degree and/or equivalent experience. May be required to become First Aid/CPR certified. May be required to become certified in overdose prevention. Bi-lingual preferred. Compensation and Status

Compensation:

$45,800 – $50,000 annually Status:
