Schedule Full‑Time / Hourly Dayshift Monday – Friday.

Location AdventHealth Hospice Fieldwork covering parts of DuPage, Kane, Kendall, Will and Cook Counties.

The Role You’ll Contribute Responsible for performing home visits as ordered by the physician. This position prepares social histories and assesses the patient’s socioeconomic and emotional status, family relationships, and coping abilities in relation to their health status and wellness potential. Additionally, this role identifies needs, prepares, implements, and evaluates individualized Plans of Care, displaying proficiency in the referral process with appropriate follow‑up. In addition, this position provides high‑quality counseling and resource coordination, promoting a team approach in the delivery of total patient care. Completes documentation according to Agency Policy and Regulatory guidelines.

The Value You’ll Bring To The Team

Evaluates the psychosocial needs of each patient and family care unit.

Develops and implements the psychosocial plan of care in response to the needs of the patients and families.

Travels to patient residence and performs initial assessment. Identifies and assesses the psychosocial needs of the patient and family, and the caregiving relationships and dynamics between the patient and family for the interdisciplinary care plan.

Assists the patients and families with obtaining financial resources.

Provides services as determined by the interdisciplinary team and reports back to registered nurse regarding assessment and need for other organizational resources.

Communicates with caregivers and makes referrals for intra‑organizational services and community resources such as Medicaid, Salvation Army, and food banks.

Provides education and assistance in completing Advance Directives.

Qualifications The expertise and experiences you’ll need to succeed

Masters degree in Social Work is required.

A minimum of 1 year experience in the social work field is required.

Healthcare/hospice experience required.

A clear/active Social Work license for the state of IL or license‑eligible under state law is required.

Maintain a current, valid State License in good standing with a safe driving record and a reliable vehicle.

Maintain valid personal automobile insurance.
