The CARE Outreach Specialist plays a critical role in connecting individuals experiencing homelessness to housing, healthcare, and community resources. This position builds trusting relationships through consistent outreach and advocacy while maintaining the highest level of professionalism, empathy, and confidentiality. The Specialist serves as a bridge between vulnerable residents and the complex network of services available across Marin County, advancing CAM’s mission to support dignity, stability, and community well‑being.

Essential Functions/ Responsibilities Direct Client Engagement

Conduct daily field-based outreach to individuals and families experiencing homelessness.

Build rapport and trust with clients through consistent, respectful, and compassionate engagement.

Identify and assess client needs related to housing, food, mental and physical health, employment, and safety.

Provide crisis response support as appropriate and ensure immediate safety needs are addressed.

Act as a visible representative of CAM and model trauma‑informed, person‑centered care.

Case Management & Documentation

Develop and maintain individualized service plans in coordination with the CARE Team Program Manager.

Document all client interactions, assessments, and outcomes in accordance with agency standards.

Maintain accurate records in HMIS and other required databases to support reporting and compliance.

Schedule, coordinate, and track appointments, follow‑ups, and service referrals.

Prioritize and manage caseloads effectively while maintaining confidentiality and data integrity.

Community Collaboration & Advocacy

Serve as a liaison between clients, service providers, and community partners.

Coordinate with county agencies, shelters, health providers, and local nonprofits to streamline service delivery.

Advocate for equitable access to housing and social services for vulnerable populations.

Share field observations, trends, and barriers with leadership to inform community strategies and resource allocation.

Represent CAM in community meetings and outreach events as directed.

Program Coordination & Communication

Maintain an organized daily schedule of outreach activities, check‑ins, and documentation tasks.

Communicate effectively with team members and supervisors to coordinate outreach priorities.

Participate in staff meetings, training sessions, and case conferences.

Support program reporting requirements and data collection efforts for grants and contracts.

Uphold CAM’s confidentiality and professional standards in all communications.

Contribute to a positive, solution‑oriented team culture grounded in CAM’s values of Relationship, Service, Inclusivity, and Unity.

Provide peer support and share best practices to enhance outreach effectiveness.

Participate in agency initiatives, training, and continuous learning opportunities.

Qualifications Education

A high school diploma or equivalent (GED) is required in accordance with CAM employment standards.

Associates in social services, Human Services, Public Health, or other related fields.

Experience

Minimum 2 years of experience in outreach, social services, or community engagement.

Skills and Abilities

Ability to work independently in field‑based settings while maintaining communication with the team.

Strong interpersonal and communication skills; ability to engage with people from diverse backgrounds.

Proficiency in data entry, documentation, and case tracking systems.

A strong commitment to social justice, equity and the agency’s mission to provide high‑quality programs to serve low‑income individuals and families.

Deep understanding – and ability to articulate – the role equity, inclusion, and diversity play in our work.

Software

Proficiency in Microsoft Office applications, particularly Outlook, Word, Excel, and PowerPoint.

Familiarity with HMIS (Homelessness Management Information System) or preferred similar databases.

Confidential and Sensitive Information Ability to maintain the highest level of confidentiality.
