Social workers help communities access health or social services and advocate for families and individuals facing physical or mental illness, disability, loss, unemployment, abuse, addiction, and other challenges. This position works PRN (as needed) from 8:00 a.m. -- 4:30 p.m. with weekends for staff coverage and 2 holidays per year.
Responsibilities
Manage all aspects of discharge planning for assigned patients to ensure timely discharge and connect with post-discharge care providers.
Communicate with the interdisciplinary team about the discharge planning status of all referred patients.
Educate patients, family, and physician regarding post-acute options and address freedom of choice and financial concerns.
Conduct initial and ongoing psychosocial assessments and interventions; make treatment recommendations to engage patients and families in adherence to the treatment plan by addressing social, psychological, financial, behavioral, and regulatory barriers to successful medical outcomes.
Serve as a resource for counseling and intervention related to treatment decisions and end-of-life issues.
Advocate for patients and families in cases involving child abuse and neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault.
Promote patient and family empowerment to make autonomous health care decisions and access needed services within the health care system and community.
Meet core social work competencies and adhere to regulations; stay current with regulations impacting patient care and regulatory compliance.
Support department goals and contribute to the success of the organization; serve as a preceptor, mentor, and resource to less experienced staff.
Minimum qualifications
Master's Degree in Social Work or Master’s of Science in Social Administration (MSSA).
Current state licensure as a Social Worker; eligible licensure includes LSW, LISW, LISW-S or PC.
Basic Life Support (BLS) through American Heart Association (AHA) certification as a Case Manager within one year of eligibility from an approved professional organization.
Experience in pre- and post-acute care.
Preferred qualifications
Three years of experience in healthcare systems, including exposure to pre-acute care.
Hospital/Medical Social Work experience.
Knowledge of community resources.
Physical Requirements
Requires full range of motion, manual and finger dexterity, and eye-hand coordination.
Requires standing and walking for extended periods.
Requires normal or corrected hearing and vision to normal ranges.
Sedentary work exerting up to 10 pounds of force occasionally and/or negligible to light amounts of force to move objects, including the human body.
Sedentary work involves sitting most of the time.
Personal Protective Equipment
Follows standard precautions using personal protective equipment as required.
