Physical Therapist
Gary and Mary West PACE is looking for passionate healthcare providers who want to be part of a team dedicated to enabling seniors to live in their communities as independently as possible by providing compassionate, comprehensive health and supportive care. We are a non‑profit program of all‑inclusive care for the elderly (PACE) affiliated with West Health.
Overview The West PACE Physical Therapist coordinates and provides restorative and rehabilitative physical therapy services to participants at the Center and at home. The role works closely with a multidisciplinary team, including a PACE physician, rehabilitation staff, occupational therapist, and other interdisciplinary team members, to maximize participant independence and safety.
Essential Functions
Conduct physical therapy assessments to determine care needs, preferences, and goals and participate in IDT meetings to develop care plans.
Deliver and document physical therapy interventions, including active and passive exercise, muscle re‑education, gait and functional training, and treatments such as massage, heat, sound, water light, or electricity.
Supervise therapy assistants, providing training and guidance for the Functional Maintenance Program.
Provide training to staff, participants, and caregivers on the use and care of therapeutic equipment; order, track, and manage equipment maintenance.
Remain alert to input from team members, participants, and caregivers, and update the IDT promptly of any changes in condition or functional status.
Support smooth care transitions between settings (hospitals, skilled nursing facilities, home) and participate in enrollment of prospective participants.
Participate in end‑of‑life care coordination and support.
Complete all required documentation in a timely and accurate manner; protect privacy and maintain confidentiality of all procedures and information.
Follow West PACE policies and procedures, and participate in required quality‑improvement activities, staff training, and meetings.
Perform other duties as assigned.
Education and Certification
Valid physical therapy license in California (or proof of license).
Master's degree in physical therapy, or master's degree in a related field plus a bachelor's degree in physical therapy.
Current Basic Life Support (BLS)/First Aid certification.
Current driver’s license and proof of auto insurance.
Skills and Experience
Minimum of two years’ physical therapy experience in a clinical setting with a frail or elderly population.
Knowledge and skills to treat frail, elderly participants with complex needs.
Highly motivated, self‑directed, able to execute tasks in a quickly changing environment and make sound decisions in emergencies.
Excellent clinical, organizational, and communication skills with seniors, families, and interdisciplinary team members.
Experience working with diverse backgrounds.

Working Conditions and Physical Demands
Work setting: Adult Day Health Center, primary care clinic, and participant homes with moderate noise levels and controlled temperatures.
Physical requirements: corrected vision and hearing, manual and finger dexterity, eye‑hand coordination, lifting/carrying up to 35 lb and pushing or pulling 50 lb with proper body mechanics; reasonable accommodation available.
Environmental conditions: exposure to bodily fluids, communicable diseases, medicinal preparations, and other typical healthcare hazards.
Medical Clearance Employees must have medical clearance for communicable diseases and up‑to‑date immunizations before having direct participant contact. Job offers are contingent upon a successful pre‑employment drug screen, background check, and physical assessment.
