As a regional agency of Catholic Charities of the Diocese of Rochester, Catholic Charities Family and Community Services (CCFCS) has served its community for over 100 years. We walk with people when they are at their most vulnerable, helping them towards independence and their full human potential while working in partnership with faith communities of all denominations.

Under general supervision from the Program Manager, the Post‑Resettlement Case Manager provides transitional services for refugees and other eligible participants in the New York State Refugee Resettlement Assistance Program. The role works with agency staff, partners, volunteers, and service providers to connect refugees with services to attain self‑sufficiency.

Essential Duties and Responsibilities

Provides appropriate information in response to queries regarding program services.

Screens potential clients for eligibility for state, federal or other programs.

Provides intake and completes program applications.

Conducts need assessments for eligible clients.

Provides referral to appropriate departmental, agency, or external programs.

Provides direct assistance services including counseling, advocacy, intervention, facilitation, transportation, and other program‑allowable services.

Documents service provision through case log entries, maintains client casefiles, and secures appropriate documentation per program requirements.

Enters and updates client and program data in program, NY state database, and other tracking platforms.

Participates in regular program quality assurance practices and keeps track of quality assurance activities.

Assists the program in preparing for internal and funder monitoring.

Adheres to agency case management principles, resettlement program service delivery philosophy, and internal program manuals.

Assists in and submission of required reports.

Maintains confidentiality of all client records per HIPAA requirements.

Participates in strategic planning, program development, meetings, and staff training activities.

Is actively involved and committed to accessing community resources.

Supports the agency’s cultural competence goals, efforts, and activities.

Actively pursues development of professional competencies through reading, supervision, in‑service training, and attendance at conferences and workshops.

Supports the Agency’s Mission, Vision, Values, and Strategic Plan, including core competencies related to being collaborative, community focused, innovative, entrepreneurial, culturally competent, and diverse.

Other duties as assigned.

Qualifications Education:

Bachelor’s Degree in Social Work, Human Services, or related field. Equivalent combination of education and experience will be considered.

Credentials:

Valid, clean NYS Driver’s License and reliable vehicle.

Experience:

At least one year in human services or case management, preferred experience with volunteers, refugees, and local providers. Ability to speak English and at least one additional language is also preferred.

Physical Demands/Work Environment:

Frequently required to sit, talk, hear, stand, walk, use hands to type, and perform light lifting. Requires close and vision with ability to adjust focus. Noise level is usually quiet.

Compliance:

Adheres to all applicable federal and state laws and regulations, including confidentiality, privacy, program, billing and documentation standards, and CFC’s corporate compliance & ethics program.
