What You’ll Do

Accountable for patient outcomes and KPI’s for a panel of CKD and ESKD patients; collaborate with external providers to align goals, treatment, and care plans.

Obtain patient history, perform physical exam, order and interpret diagnostic tests, and formulate short‑term and longitudinal care plans.

Present patient cases and provide clinical support during rounds and interdisciplinary team meetings.

Ensure care plans follow best practices for chronic kidney care, including CKD stages, comorbidities, ESKD transitions, and shared decision‑making.

Serve patients in multiple settings—home, clinic, partner MD space, and via telehealth/virtual visits.

Liaise with dialysis facilities, PCPs, and nephrologists to keep care plans updated and streamline transitions.

Maintain current board certification and state‑specific continuing‑education requirements.

Adhere to Strive’s documentation policy and deliver primary and urgent visits to adult and geriatric patients with CKD.

Perform in‑person care that may require standing, sitting, walking, pulling, pushing, and lifting.

Minimum Qualifications

Master’s degree in Nursing or similar field.

2+ years experience as a Family NP, Primary Care NP, or in internal medicine, cardiology, nephrology, or case management.

Current state licensure required.

National Board Certification.

DEA license required or eligible to obtain within 90 days of hire.

Current BLS or CPR certification required.

Reliable transportation, active driver’s license, and ability to travel across the assigned region for patient visits.

Fast‑connect internet with minimum speeds 3.8 Mbps/3.0 Mbps and latency Preferred Qualifications

Basic computer and Microsoft Office skills.

2+ years experience with Electronic Health Records (EHR) preferred.

Experience using audio‑visual technology platforms.

Excellent communicator, team builder, and evidence of results.

Knowledge of data and experience managing clinical, financial, and patient satisfaction outcomes.

Exceptional at building strong patient and family relationships centered on engagement, trust, empathy, and doing what’s best for the patient.

Annual Salary Range:

$118,000.00 – $149,600.00
