What You’ll Do

Manage patient care for CKD and ESKD patients, collaborating with interdisciplinary team members.

Obtain patient history, perform exams, order and interpret tests, develop individualized care plans.

Present patient cases, contribute to clinical rounds and interdisciplinary meetings.

Apply best practices for chronic kidney care, shared decision‑making, and end‑of‑life planning.

Serve patients in various settings (home visits, clinics, partner MD spaces, telehealth).

Act as liaison with dialysis facilities, PCPs and nephrologists to coordinate transitions.

Maintain current board certification, state licensing, DEA and BLS/CPR certifications.

Adhere to Strive’s documentation and policy expectations.

Minimum Qualifications

Master’s degree in Nursing or equivalent.

2+ years experience as Family NP, Primary Care NP, or experience in internal medicine, cardiology, nephrology, or case management.

Current state licensure and national board certification.

Current DEA license or eligible to obtain within 90 days.

Current BLS/CPR certification.

Valid driver’s license with reliable transportation for patient travel.

Preferred Qualifications

Basic computer and Microsoft Office skills.

Experience with Electronic Health Records (EHR).

Wound care certification.

Experience with audio‑visual technology platforms.

Proficient in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of complex chronic patients.

Excellent communicator, team builder, with evidence of results.

Strong data‑driven approach to managing clinical, financial, and patient satisfaction outcomes.

Committed to building strong patient and family relationships centered on engagement, trust, empathy, and follow‑through.

Annual Salary Range $118,000.00‑$149,600.00
