Responsibilities Reporting to the Coordinator of Care, you will be responsible for seeing patients in their homes for revisits based on skilled need, implementing the plan of care, evaluating treatment, and documenting visits electronically within 24‑48 hours.
Qualifications
NY State LPN license required
Minimum of one year med/surg experience in a hospital or long‑term care facility
NY State driver’s license and owned vehicle
Good computer skills
