Enrollment Nurse, Remote - Spring Valley (Biotech)
Job Overview
The Enrollment Nurse is responsible for completing the UAS assessment, reviewing the NYIA assessment, creating the time‑tasking tool for the potential member, and developing the Person‑Centered Service Plan (PCSP). The Enrollment nurse also conducts PCSP meetings with the potential member and obtains enrollment documentation to ensure the member enrolls into the plan.
Conducts UAS‑NY assessment visits for potential plan members, assesses newly referred members, evaluates overall health needs and eligibility for enrollment, provides member and family teaching at initial visits, and follows up with intake/enrollment and care management teams regarding eligibility status.
Initiates and coordinates a safe plan of care with member/family to meet all needs; documents UAS‑NY and visit notes, progress notes, and PCSP; communicates member needs and identified care plans to care managers. Attends professional in‑service programs, staff meetings, and participates in the agency QA program. Covers shifts for other nurses as necessary, performs assessments and completes documentation within time frames, attends mandatory staff meetings, meets productivity requirements, travels as required, and undertakes other assignments within the RN scope of practice.
Specific Knowledge, Skills, and Abilities
Knowledge of NYS requirements specific to MLTC enrollment and marketing guidelines (preferred).
Demonstrates excellent communication, negotiation, organizational and public relations skills.
Proficiency in the use of office equipment, Microsoft Office Suite, and Electronic Health Records system.
Demonstrates autonomy, assertiveness, flexibility and cooperation in performing job responsibilities.
Essential Responsibilities

Attendance at mandatory staff meetings and professional development.
