Overview We are seeking compassionate and experienced

Registered Nurses (RNs)

to join a

Managed Long Term Care (MLTC)

program as

Intake Nurses . This is a

full-time opportunity

offering competitive pay and a

$20,000 sign-on bonus .

The Intake RN will assess potential members for eligibility, educate families on the program, and ensure a smooth transition into managed long‑term care. Training and support are provided to ensure success in this community‑based role.

Key Responsibilities

Conduct in‑home

UAS‑NY assessments

to determine MLTC eligibility and safety.

Educate potential members and families on available program benefits and services.

Collaborate with intake, outreach, and clinical teams to ensure timely and appropriate service delivery.

Maintain thorough and accurate documentation per state and agency standards.

Build and maintain strong relationships with referral sources, members, and care teams.

Qualifications

Active NYS RN license

(required).

Associate’s Degree in Nursing (ADN)

required;

BSN preferred .

Minimum 1 year of acute care experience

required.

UAS‑NY experience required.

Home care or MLTC experience preferred.

Proficient in MS Office.

Excellent written, verbal, and interpersonal communication skills.

Bilingual (Spanish or Chinese)

preferred.

Salary:

$95,000 – $105,000 annually

Sign‑On Bonus:

$20,000 (paid in $5,000 installments every six months, less taxes)
