Base Pay Range:
$110,000.00/yr – $115,000.00/yr
Position Summary:
Community of Hope (COH) seeks an Associate Director, Nursing Care Management to lead the Nursing Care Management Program. The role focuses on ensuring patients with chronic conditions or special health needs receive compassionate, coordinated care, inspiring the team, strengthening quality outcomes, and reducing unnecessary hospital visits through proactive, patient‑centered support.

What You’ll Do
Lead the nurse care management team in managing the effectiveness of the care management service delivery model, including evaluation of program outcomes and individual staff productivity and performance.
Ensure that patients with chronic diseases or unnecessary utilization are adequately monitored and supported using population health management tools; identify gaps in services and assist in developing plans and partnerships to meet patient needs.
Oversee coordination of transitions between healthcare providers and settings to reduce emergency department and inpatient admissions, readmissions, and length of stay.
Maintain regular communication with panel of care‑managed patients via face‑to‑face or telephone encounters based on level of acuity and program‑specific requirements, and via the patient portal.
Facilitate ongoing communication among practice and care teams by ensuring participation in huddles, hosting regular conference calls, in‑person meetings, or coordinating regular email updates to ensure alignment of activities and exchange of information across the care teams.
Coordinate with the Nurse Director of Clinical Services, Medical Directors, and departmental leads as needed, including management of clinically related patient complaints, unusual incident reports, HIPAA, and OSHA incidents.
Must-Haves
Bachelor of Science degree in nursing.
Current, unencumbered DC Registered Nurse license with current BLS certification.
Knowledge of primary care and health maintenance.
One year of management experience.
Proficiency with computers and electronic health records.
Strong verbal and written communication skills.
Strong organizational skills with the ability to multitask.
Demonstrated cultural competence in communicating with low‑income populations.
Team‑orientated collaborative approach.
Ability to travel between COH sites or relocate to a different COH site on a full or part‑time basis.
Ability to work Saturdays and evenings.
Nice-to-Haves
Three years of care management experience.
Experience educating patients with chronic health conditions.
Bilingual in Spanish, Amharic, or French.
Familiarity with eClinicalWorks.
Nonprofit or mission‑driven healthcare experience.
