The Division of Correctional Health Services (CHS)
of New York City Health + Hospitals (H+H) is responsible for the delivery of medical care, mental health care, and discharge planning in the jail system. Approximately 25% of new admissions per year to the NYC jail system will receive care on the mental health service. This service strives to provide safe, holistic and evidence-based therapeutic interventions for the diverse range of mental health issues that face the population, from serious mental illness to the natural struggles that accompany incarceration. Responsibilities: Facilitate Telehealth encounters as needed. Administer total care (bed bath, partial or complete). Assist with patient’s hygiene – including skin care, mouth care, shaving, etc. Transfers patients – in and out of bed, stretcher, and wheelchair. Positions patients and carry out a range of motion exercise. Prepares patients for meals and assists with feeding as needed. Assists with unit patient admissions and transfers. Measures & records patients’ vital signs (such as height, weight, temperature, blood pressure, pulse, respirations, etc.) in the electronic health record. Perform EKGs as required and follow outlined procedures for submitting abnormal EKGs. Perform dry sterile dressing changes and assist with other patient treatment. Administers hot and cold applications to patients, under supervision. Perform blood draws and urine testing. Chaperone staff in all clinical areas when requested. Perform tasks consistent with the PCA job description. EMERGENCY RESPONSE Responding to/addressing medical emergencies, such as falls, heart attacks, strokes, burns, and recovering post-operative patients. Provide clinically appropriate care as directed by the Registered Nurse and/or Physician. When clinically indicated, provide care, such as CPR, AED, prevent shock, control severe bleeding, prevent spinal damage, etc. Monitor patients’ condition and keep them stable while en route to the clinic. Aid in preparing patients for transfer to the emergency units or hospitals. Document the medical treatment that is given to the patient. Keep the equipment clean after use; check and replace damaged and used supplies. Collaborate with the pharmacy to replenish supplies. PHYSICAL DEMANDS Ability to lift fifty (50) pounds. Moving, lifting, or transferring patients may involve lifting up to 100 pounds. Ability to stand for extended periods. Fine motor skills. Visual acuity. Schedule: Sun, Mon, Wed, Thu, Sat 4pm-12am. Scheduling note: PCAs are required to work every other weekend. This position is unionized through 1199 and granted 1199SEIU benefits. Qualifications: A four-year high school diploma or equivalent (GED) required. NYS Department of Health phlebotomy training program. Patient Care Associate with EKG Certification.
