VA Detroit Summary This position is eligible for the
Education Debt Reduction Program (EDRP) , a student loan repayment program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program approval, award amount (up to $200,000), and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants are ineligible to apply. Duties Serve as Medical Director for Home-Based Primary Care Program (HBPC) and Geriatric Care programs, overseeing leadership, program management, quality assurance, and performance improvement. Manage a panel of Veteran patients with complex comorbidities within the Home-Based Care program. Provide oversight of the Geri-PACT Consultation Clinic and Geriatric Medicine Fellows. Act as the subject matter expert for home-based and community-based care services for Veterans. Supervise and evaluate HBPC medical providers in Oklahoma City and Lawton, OK, which may require travel to Lawton. Lead planning, quality improvement, and program development for HBPC programs. Serve as Chair of the HBPC Committee and conduct monthly meetings. Manage and report data related to the HBPC program, including clinical and administrative data. Supervise new Geriatric and Home-based Care providers and Fellows at the Oklahoma City VA and Lawton VA facilities. Engage in Fellowship training and maintain a University affiliation.
