Staff Nurse - RN Location: Harbor Community Health Center – Hyannis, MA.

Hours: 32 hours per week (Mon/Wed/Fri 8:00 AM–5:00 PM; Thu 8:00 AM–3:00 PM; Sat rotation 8:00 AM–12:00 PM every 8‑10 weeks).

Salary: $38.03–$57.07 hourly (minimum $40.00). Sign‑on bonus: $5,000.00.

Responsibilities

Performs routine nursing care to patients, following age‑specific guidelines and standard precaution practices.

Assists the care team and/or provider in developing and implementing a comprehensive plan of care.

Administers medications and vaccinations via oral, topical, subcutaneous, intradermal, and intramuscular routes per provider orders or standing orders.

Documents patient care activities, assessments, treatment plans, and interventions in the electronic health record.

Verifies that emergency equipment is maintained and working once per month or after use.

Participates in team, department, and clinic meetings to support process improvement and contribute to care plans.

Provides cross‑coverage and training for staff in similar roles as needed.

Delivers health coaching and education for common conditions (e.g., asthma, hypertension, diabetes) through individual, group, and co‑visit sessions.

Ensures patients stay up to date with clinical care guidelines, immunizations, and health maintenance tests.

Supports transitions of care through post‑ED/hospitalization visits, as appropriate.

Qualifications

Successful completion of a nursing program (RN); BSN preferred.

Current Massachusetts RN license and BLS certification.

Experience in a community health setting preferred; ambulatory care or health insurance environment experience a plus.

Basic computer literacy; proficiency with Microsoft Office 365 and Electronic Medical Record systems (EPIC/OCHIN preferred).

Excellent customer service skills and strong verbal and written communication.

Ability to work effectively within a team and independently.

Ability to work with economically, racially, and linguistically diverse patients.

Bilingual a plus: English and Vietnamese/Spanish/Portuguese or Haitian Creole.
