POSITION: Care Partner
TITLE OF IMMEDIATE SUPERVISOR: Health and Wellbeing Director
DEPARTMENT: Resident Care
STATUS: Non-Exempt/Hourly
SUMMARY
This position will provide personal care service and engagement with residents and perform related services at the community in accordance with the plan of care assigned by the primary licensed nurse. This position reports directly to the Health and Wellbeing Director.
QUALIFICATIONS AND EDUCATION REQUIREMENTS
High school diploma or GED
Successful completion of a state approved nursing assistant training program preferred
Current, active certification on the Georgia Nursing Assistant Registry preferred
Must successfully clear a background check, TB skin test or x-ray, and health screening
Current CPR certification
Current certificate of training for successful completion of the five (5) hour initial fire safety training provided by the Rules and Regulations of the Georgia Safety Fire Commissioner
PRINCIPLE DUTIES AND RESPONSIBILITIES
Provides assistance with assigned personal care tasks that meet resident physical and emotional personal needs consistent with the Orchard philosophy.
Assists or leads small groups with engaging in one on one experiences with residents.
Responsible for assigned resident care tasks, ensuring accuracy, completeness and compliance with regulations.
Resident care tasks include maintaining personal hygiene, including bathing, oral care, and skin and nail care.
Answers residents' call signals and assists with toileting.
Observes residents' conditions and reports changes to professional staff.
Provides resident care by applying dressings and supervising exercise routines.
Transports residents to and from dining room using a wheelchair when applicable.
Uses aseptic technique for infection control.
Documents accurately all care given.
Participates in the planning of resident care through shift reports and staffing meetings.
Encourages/assists residents in keeping their rooms clean and orderly.
Creates and maintains an atmosphere of warmth and personal interest.
Maintains the dignity of the resident and provides person-centered care.
Consults and collaborates with the interdisciplinary team.
Demonstrates effective communication and conflict resolution skills.

Physical Requirements
Lifting: More than 25 lbs, routinely; More than 50 lbs, occasionally.
Transfer: 150 lb. or greater resident weight.
Position requires close work; finger dexterity; adequate hearing and vision.
Cognitive or Mental Requirements
Critical thinking, reading, writing, and mathematics.
Clear verbal articulation of the English language.
Working Environment
Indoors with possible exposure to communicable diseases.
May be subject to hostile and emotionally upset residents.
