Job Title:

Community-Based Housing Case Manager

Schedule:

40 hours, Sun–Thu, 8:30 am – 5:00 pm

Pays:

$23.48 – $36.00 per hour, DOE (salary ranges based on experience and skill set)

Retention Payment:

$3,000.00 ($1,000.00 payable at 12 months, $2,000.00 at 24 months)

Role Overview:

Supportive Housing provides comprehensive, intensive case management support to homeless and chronically homeless individuals in housing using the Housing First model, promoting long-term stability and ending homelessness in Boston. The Case Manager works as part of an intensive case management team serving tenants who may have untreated mental illness, substance use disorders, and other health challenges. The case manager applies a harm‑reduction, trauma‑informed approach and utilizes Stages of Change Theory and Motivational Interviewing techniques. Responsibilities include managing a specific caseload of tenants and collaborating with other staff on program‑wide efforts to stabilize and promote tenant growth. Additional duties include: assisting tenants with program documentation; helping tenants obtain benefits, health care, education, employment, and legal aid; supporting development of daily living skills; creating and maintaining individual service plans and documentation.

Responsibilities

Manage a caseload of tenants.

Assist tenants with program documentation.

Help tenants obtain benefits, health care, educational opportunities, employment opportunities, and legal aid.

Support the development of tenants’ activities of daily living skills.

Create and maintain individual service plans and necessary documentation.

Collaborate with other Supportive Housing staff on program‑wide efforts.

Qualifications

High School Diploma or GED.

Strong written communication skills.

Computer proficiency with Microsoft Office.

Experience working with individuals experiencing substance abuse, mental illness, trauma, homelessness and/or other complex disorders.

Commitment to learning and using Harm Reduction and Motivational Interviewing skills.

Knowledge of effective crisis intervention techniques and ability to respond appropriately.

Experience working as part of a team.

Preferred: bilingual (Spanish/English).

Preferred: experience working with chronically homeless individuals in Housing First programs.

Preferred: knowledge of local caregiving and advocacy resources for people with substance abuse, mental illness, trauma histories, head injury, medical illness, physical disabilities, relationship violence, criminal and civil justice system involvement.

Preferred: knowledge of government benefits systems and Boston area service providers.
