Hours/Shift: Per Diem (PRN)
Availability to work a minimum of 2 shifts per month on an "as needed" basis – shifts scheduled weekdays, 8 a.m. – 5 p.m.
Compensation: $40 per hour
Requirements:
Current licensure in a clinical discipline (RN/LPN/LVN, RT, or Social Work license per state guidelines).
Previous experience in a critical care setting.
Previous discharge planning experience highly preferred.

Role Summary:
The Case Manager is responsible for utilization reviews, resource management, discharge planning, treatment plan management, financial management, and medical record documentation, reporting to the Director of Case Management and providing social work services as required.
Responsibilities
Develops and implements patient‑specific, safe and timely discharge plans.
Performs verification of utilization criteria reviews.
Builds relationships and coordinates with payor sources to assure proper reimbursement, focusing on resource management.
Demonstrates compliance with facility‑wide Utilization Management policies and procedures.
Coordinates UR compliance with Quality Management to assure all licensure and accrediting requirements are fulfilled.
Maintains fiscal responsibilities, identifying and negotiating reimbursement to maximize insurance benefit coverage; reviews insurance verification forms to minimize risk.
Facilitates multidisciplinary team meetings with physicians, nurses, RTs, and rehabilitation therapists.
Minimum Requirements
Current licensure in a clinical discipline (RN/LPN/LVN or RT or Social Work per state guidelines).
Previous inpatient hospital experience in critical care or acute care (ICU, step‑down, med‑surg, vents).
Adequate experience in acute medical case management, confident managing and directing a plan of care for chronically critically ill populations.
Preferred Qualifications
Specific experience in care management and discharge planning.
Working knowledge of the insurance industry and government reimbursement.
Availability to work a minimum of 2 shifts per month on an "as needed" basis – weekdays 8 a.m. – 5 p.m.
