Job Description
First Stop Health is seeking a Part‑Time Registered Dietitian, Certified Diabetes Care and Education Specialist who is detail‑oriented and passionate about helping patients improve nutrition status, blood glucose control, prevent complications, and enhance overall health. In this role, you will provide expert nutrition counseling, diabetes self‑management education, and lifestyle coaching to individuals and groups, collaborate with a multi‑disciplinary team, and contribute to clinical operations. You will report directly to the Director of Health Coaching.
The ideal candidate is passionate about advancing the company’s Quadruple Aim: enhancing patient care experiences, improving population health outcomes, reducing costs, and bolstering clinician well‑being. They are enthusiastic, service‑oriented, curious, and collaborative.
Schedule Expectations
15‑20 hours per week
2 hours per week must be between 4‑8 pm CT Mon–Thu or 8 am‑12 pm CT Sat–Sun
Schedule needs to be consistent weekly
Responsibilities
Support a roster of patients virtually/telephonically (100% remote from home)
Utilize established tools and protocols to assess patient health status and identify health risks
Document goals, behavioral and clinical outcomes, and administrative details of all registered dietitian interactions using the portal
Navigate patients to helpful resources and tools (referrals, health information, etc.) that are made available to you
Conduct comprehensive nutrition assessments, considering medical history, lab values, medications, lifestyle, and cultural preferences
Develop individualized meal plans and nutrition interventions aligned with evidence‑based guidelines
Provide diabetes self‑management education and support (DSMES), including carbohydrate counting, insulin management, glucose monitoring, and problem‑solving skills
Content creation and delivery for customer requests such as webinars and speaking engagements
Support implementation of new products/technology
Ensure outcome collection and analysis
Prioritize whole‑person care by collaborating with cross‑disciplinary teams
Maintain and improve processes by following standards, policies and procedures, and recommending new approaches
Requirements
Registered Dietitian (RD/RDN) credential through the Commission on Dietetic Registration (CDR)
Licensed in your state of residence
Certified Diabetes Care and Education Specialist (CDCES) credential from the Certification Board for Diabetes Care and Education
5+ years of experience preferred
Bachelor’s or Master’s degree in nutrition, dietetics, or a related field
Strong knowledge of nutrition therapy for type 1, type 2, gestational diabetes, prediabetes, cardiovascular and metabolic health, women’s health and nutrition through the lifecycle
Excellent communication, motivational interviewing, and behavior change counseling skills
Fervor for cultural curiosity and experience engaging and working with people across a wide range of social, cultural, and socioeconomic backgrounds
Self‑starter
Flexible and eager to learn new skills
Ability to work independently in a deadline‑oriented environment
Fluency in basic computer skills including Microsoft 365 and EHR systems
