Position Summary:
POD Health is seeking a Licensed Clinical Social Worker (LCSW) to join our clinical team full‑time, on‑site in Brooklyn, NY. In this role, you’ll support patients by assessing mental health needs, developing care plans, and collaborating with a multidisciplinary team to ensure well‑rounded support for conditions such as addiction, depression, and anxiety.
Schedule:
Monday – Friday | 9:00 AM – 5:00 PM | On‑site in Brooklyn, NY 11218

Base pay range $50.00/hr - $70.00/hr

Key Responsibilities
Conduct thorough biopsychosocial assessments and develop personalized care plans.
Focus on treating addiction, depression, and anxiety, primarily in senior populations.
Liaise closely with the medical team to define care goals and manage patient outcomes.
Educate patients about available services and promote active participation in their care.
Maintain timely and accurate documentation in the Electronic Health Record (EHR).
Participate in weekly clinical supervision and interdisciplinary team meetings.
Uphold NASW ethical and professional standards in all areas of practice.
Build therapeutic rapport with patients from diverse backgrounds while maintaining clinical boundaries.
Required Qualifications
Master’s degree in Social Work from an accredited institution.
Current LCSW license in New York State required.
Experience supporting patients with addiction, depression, and anxiety.
Strong interpersonal, communication, and documentation skills.
Ability to work independently and collaboratively within a team‑based model.
Commitment to HIPAA compliance and maintaining confidentiality.
Passion for serving aging and diverse communities.
Preferred Qualifications
Bilingual in Spanish or Chinese.
Familiarity with telehealth platforms or virtual care settings.
