Pay Range
Base pay range: $37.00/hr - $44.00/hr. Individual compensation within this range is based on a candidate’s specific qualifications, including education, overall experience, relevant experience, and specific skills. RN Home Health Per Diem (Paid per hour)
Position Summary For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness, and health care services. The Registered Nurse (RN) – Home Support assumes responsibility for an assigned group of residents requesting nursing care interventions. The RN – Home Support/Private Home Care completes daily caseload assignments in a timely manner including all documentation. This position functions in compliance with State and Federal Regulations, North Hill’s policies, procedures, and standards.
Job Specific Duties
Implements a plan to provide patients and family/caregivers with needed education to manage care needs, as requested.
Evaluates resident progress towards goals of care and revises plan accordingly in consultation with Supervisor.
Processes lab requests, including following orders for labs, completing requisitions as needed, tracking results, communicating and implementing follow‑through with physicians and patients as needed.
Responds to Personal Emergency Response System calls and non‑Personal Emergency Response System emergency and emergent calls, in conjunction with security. Triages competing calls in the event of multiple calls, requesting other nursing assistance as needed.
Communicates concerns with immediate supervisor and works with the interdisciplinary team to meet resident needs.
Anticipates potential problems, implements interventions and communicates per documentation guidelines.
May complete Skilled Nursing visits for the certified home health agency as needed, including documenting the skilled nursing visit note in the electronic health record and all other documentation necessary to complete visit, following completion of required competency review.
Follows all Emergency Management Plans and actively participates in fire and evacuation drills.
Meets department uniform, appearance and grooming requirements.
Complies with all OSHA safety regulations, local applicable laws regarding health, safety or environment.
Supervisory Responsibilities None

Ability to maintain confidentiality and strong customer support skills.
Complies with protection of health information (HIPAA) safeguards.
Ability to work independently with limited supervision.
Work professionally with people from a wide variety of cultural and socio‑economic backgrounds.
Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public.
When working in Resident areas, must be able to communicate effectively in English.
Attends and participates in in‑service educational classes, Healthcare Academy and staff meetings as directed.
Education And Experience
Bachelor of Science in Nursing, Registered Nurse preferred.
Professional license in good standing in the Commonwealth of Massachusetts.
Ability to read, write and understand English.
2 years’ experience as a Registered Nurse, preferably in a home health care setting.
Proficient with computer software programs.
Physical Requirements
Must be able to move intermittently throughout the workday.
While performing the duties of this job, the Team Member is regularly required to talk and hear. The Team Member frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
Must meet the general health requirements set forth by the policies of this community.
Must be capable of lifting up to 50 pounds.
Work Environment: Will be subject to frequent interruptions.
May be asked to work beyond normal working hours and to work in other areas temporarily, when necessary.
May be subject to call‑backs during emergency conditions (e.g., severe weather, evacuation, post‑disaster).
This position requires regular weekend and holiday work.
