Job Title: PRN Oncology Social Worker

Overview Join Sibley as a PRN Oncology Social Worker, providing essential counseling and support services to cancer patients and their families in an outpatient setting. Collaborate with healthcare professionals to address psychosocial issues and connect patients with vital community resources.
Key Responsibilities
Deliver compassionate counseling to patients and families throughout their cancer journey, based on referrals.
Assess and identify psychosocial needs through collaboration with the Cancer Committee and physicians.
Facilitate support groups and participate in Cancer Center events to enhance the psychosocial well‑being of patients.
Create and coordinate discharge plans for patients with complex needs, working closely with medical teams and families.
Provide consulting support for inpatient discharge planning, ensuring continuity of care.
Document all services in medical records and coordinate grant‑funded transportation options.
Serve as a liaison for post‑treatment services with external providers and payors.
Identify patients with unique needs and facilitate referrals for specialized services and financial assistance.
Conduct educational sessions for staff on oncology social work topics.
Qualifications
Master's degree in Social Work from an accredited institution.
Licensed to practice as a Social Worker in Washington, D.C.
2‑5 years of experience in a healthcare setting, preferably oncology.
Current CPR certification.
Skills
Effective communication with healthcare teams and patients.
Knowledge of community resources and age‑specific care needs.
