Position Overview We are seeking an experienced, self‑driven
Full‑Time Physical Therapist (PT)
for Home Health in San Jose, CA. As a PT, you will evaluate patients, develop individualized treatment plans, and improve mobility, strength, and quality of life. You will collaborate closely with our Clinical Director and Clinical Staff.

Responsibilities
Provide physical therapy services within scope and individualized plan, in coordination with the Interdisciplinary Team.
Administer treatments, monitor patient conditions, and report changes to Physician, Case Manager, or Director of Nursing.
Maintain accurate clinical records and submit documentation per company policy using a company‑provided device.
Comply with all company policies and procedures to ensure high standards of care.
Requirements
Current Physical Therapist (PT) license to practice in California – required.
Experience in Home Health or Hospice care – 1 year required.
Current CA driver’s license.
Able to travel within a 15‑25 mile radius on a regular basis.
