Join a team that’s redefining senior primary care in a value‑based environment. Work alongside nurse practitioners, pharmacists, care coaches, behavioral health specialists, and more to deliver personalized, high‑quality care to seniors.

Pay Range Base pay: $219,400 – $306,900 per year. Compensation may exceed $300K base plus quality metric bonus; sign‑on bonus up to $60K.

Responsibilities

Provide comprehensive care to seniors in accordance with clinical standards.

Collaborate daily with your care team to ensure coordinated, high‑quality care.

Manage referrals, hospital/SNF coordination, durable medical equipment, and home health services.

Participate in daily huddles (clinical assistants often lead).

Document care accurately and efficiently with support from quality‑based coders.

Participate in on‑call rotation and support growth initiatives at your center.

Ensure compliance with all licensing and accreditation requirements.

Spend 100% of your time clinically focused on direct patient care, including charting, meetings, and general admin that relates to patient care.

Qualifications

Graduate of an accredited MD or DO program.

Board certification or eligibility to become certified (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine.

Current and unrestricted medical license, or willingness to obtain a license in the state of practice and other regional states as required.

Excellent verbal and written communication skills.

Strong interpersonal relationships with colleagues and patients.

Fully engaged in the concept of an integrated team‑based care model.

Willingness and ability to learn and adapt to value‑based care practice.

Superior patient/customer service orientation.

Basic computer skills, including email and EMR.

Participation in TB screening program required; screening for TB mandatory if hired.

Preferred Experience

Active and unrestricted DEA license.

Medicare Provider Number.

Medicaid Provider Number.

Minimum of 2–5 years directly applicable experience preferred.

Experience managing a Medicare Advantage panel and understanding best practices in coordinated care within a value‑based environment.

Knowledge of Medicare guidelines and HEDIS quality indicators.
