Location: St Anthony's Hospital – St Petersburg, FL
Schedule: Every other Saturday & Sunday, 8:00 AM – 4:30 PM (Part-time, 16 hours per week)
Responsibilities
Provides linkage, monitoring, planning and advocacy for our population to achieve and maintain maximum functioning.
Possesses clear understanding of available resources to promote quality cost-effective outcomes.
Effectively maintains and manages assigned caseload based on individual's strengths, barriers, and preferences.
Coordinates discharge planning through assessment, referral, and interdisciplinary planning.
Adjusts to changing priorities in a fast-paced environment.
Position Details
Location: St Anthony's Hospital – St Petersburg, FL
Status: Part-time, 16 hours per week
Schedule: Every other Saturday & Sunday, 8:00 AM – 4:30 PM
Weekend Requirement: Every other
On Call: No
Requirements
Certification: RN (Registered Nurse) – Required; ACM (Case Management) – Preferred; CCM (Case Manager) – Preferred.
Education: Associates of Nursing – Required; Bachelor of Nursing – Preferred.
Experience: 2 years Acute Care; 2 years Registered Nurse – Required.
