Physical Therapist (PT) – Outpatient & Senior Care
Location: Preston Greens
Job Type: Full-Time
Schedule: Monday to Friday, 9:00 AM – 4:30 PM
Thrive Therapies is expanding and seeking a dedicated Physical Therapist to join our team. We are looking for a highly motivated individual with a passion for outpatient care, strong work ethic, and commitment to our core values of Quality, Integrity, and Passion. Thrive Therapies offers services on-site at a senior living facility in the Hamburg area of Lexington, KY called Preston Greens.
Comprehensive Patient Care:
Provide assessments, diagnoses, and tailored treatment plans for adult and senior populations. Treatment Implementation:
Develop and implement effective care plans using evidence-based practices, including discharge planning and guiding therapy sessions based on anatomy and physiology knowledge. Documentation & Compliance:
Maintain accurate medical records and documentation in compliance with Medicare and other provider guidelines. Communication & Advocacy:
Effectively communicate with patients, families, and healthcare teams regarding plans of care, advocating on behalf of patients for optimal results. Team Collaboration:
Work collaboratively with the administrative team, taking initiative in managing caseloads, obtaining necessary orders, and securing authorizations. Workflow Improvement:
Problem-solve to enhance company workflow and potentially teach relevant exercise classes.
Qualifications & Skills Graduate of an accredited Physical Therapy program. Current and active Kentucky PT License; adherence to all KY continuing education requirements. CPR certified. Experience working with seniors in outpatient, home health, or senior living settings.
