Senior Care of Marion is seeking a dedicated and compassionate Physical Therapist to join our healthcare team. The ideal candidate will provide exceptional patient care across various settings, including inpatient, outpatient, home health, and community care. This role involves developing and implementing personalized care plans to improve mobility, manage pain, and enhance the quality of life for our diverse patient population, including geriatrics, pediatrics, and orthopedic cases. The Physical Therapist will collaborate with multidisciplinary teams to ensure comprehensive discharge planning and effective treatment strategies.
Duties
Conduct thorough assessments of patients’ physical conditions, including physiology and anatomy evaluations.
Develop individualized care plans tailored to patient needs, incorporating physiotherapy techniques such as trigger point therapy and orthopedics.
Administer treatments in various settings—hospital, outpatient clinics, home health, and community environments—adapting approaches as necessary.
Monitor patient progress and adjust treatment plans accordingly to optimize recovery outcomes.
Educate patients and caregivers on exercises, mobility techniques, and injury prevention strategies.
Document patient interactions accurately using EMR systems in compliance with healthcare regulations.
Coordinate discharge planning to ensure seamless transition from hospital or clinic to home or community care.
Collaborate with physicians, nurses, and other healthcare professionals to provide integrated patient care.
Stay current with advancements in physiotherapy practices and maintain professional licensure requirements.
Qualifications
Valid state licensure as a Physical Therapist.
Proven experience in inpatient, outpatient, hospital, or home health settings; experience in geriatrics, pediatrics, orthopedics preferred.
Strong knowledge of physiology, anatomy, medical terminology, and care plan development.
Familiarity with EMR systems and documentation standards.
Excellent patient care skills with a compassionate approach tailored to diverse populations including seniors and children.
Ability to perform discharge planning effectively across acute care and community settings.
Knowledge of physiotherapy techniques such as trigger point therapy and orthopedic rehabilitation.
