Transition Specialist
within our Vocational Program, you will play a key role in helping adults develop the skills, confidence, and real‑world experience they need to move toward competitive, integrated employment.
This role is perfect for someone who enjoys teaching, mentoring, being in the community, and fostering personal growth in others.
What You’ll Do As a Transition Specialist, you will support individuals through structured, goal‑based activities designed to promote independence, employability, and community involvement:
Skill‑Building & Participant Support
Teach
daily living skills, employability skills, social skills, and problem‑solving strategies
Implement structured activities and curriculum that support readiness for employment
Adapt support based on each participant’s strengths, needs, and barriers
Encourage positive social behavior, independence, and personal choice
Community‑Based Learning
Support participants at
community job sampling sites, volunteer placements, and other integrated settings
Build and maintain partnerships with
local businesses and community organizations
Transport participants to and from community‑based sites using agency vehicles
Program Planning & Documentation
Maintain knowledge of each participant’s
Individual Service Plan (ISP)
Review ISPs at least annually and recommend updates based on progress
Assist individuals in identifying personal goals and strategies for achieving them
Complete required documentation including daily logs, critical logs, medication records, and incident reports
Implement behavior support plans and provide crisis management when necessary
Health, Safety & Compliance
Administer medications according to agency standards
Follow all treatment plans including dietary, behavioral, and adaptive equipment needs
Ensure participant rights, confidentiality, and safety at all times

Bachelor’s degree
in Human Services or related field (required)
Valid NJ Driver’s License
with no more than
5 points
Strong verbal and written communication skills
Ability to problem‑solve, work independently, and collaborate with a team
Comfortable networking and building relationships with community partners
Ability to meet physical requirements (including lifting 25+ lbs, assisting with mobility, and navigating community sites)
