is hiring an Occupational Therapist to provide individualized care in clients’ homes. The role is full‑time, located in New York, NY, and offers a competitive salary with benefits.
Responsibilities
Conduct comprehensive assessments of clients’ physical, functional, and cognitive status.
Develop and implement individualized treatment plans that include therapeutic modalities, goal setting, and progression monitoring.
Collaborate with physicians, nurses, and other health professionals to coordinate care and align interventions with medical prescriptions.
Evaluate home environments and recommend adaptive equipment or environmental modifications.
Provide patient‑ and family‑centered education on functional strategies, use of adaptive devices, and safety precautions.
Maintain accurate, timely documentation of evaluations, progress notes, and treatment plan revisions in accordance with agency policies.
Participate in interdisciplinary case conferences and contribute to care planning discussions.
Address psychological and emotional responses to recovery through counseling and supportive communication.
Assist with troubleshooting and modify treatment strategies for medically or financially ineligible referrals.
Qualifications
Valid New York State license and registration as an Occupational Therapist.
Member of the American Occupational Therapy Association (AOTA) or graduate of an AOTA‑accredited program.
Minimum of one year of experience in a home‑health or related setting.
Experience working with pediatric populations is required; experience with adult and geriatric clients is a plus.
Demonstrated knowledge of Medicare/Medicaid requirements, state regulations, and credentialing processes.
