Scope of Position: In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, family, pharmacists, nurses, physicians, medical students, undergraduate and graduate pharmacy students, and other associate hospital personnel. Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to practice area.
Position Summary: This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include, but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff).
Minimum Qualifications
Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred.
On-going responsibilities: Conducts experiential and/or didactic teaching of pharmacy students (including interns), pharmacy residents, and permanent staff. Professional instruction to the hospital and clinical staff or other professional organizations. Participates in and presents intra-departmental continuing education programs in addition to professional presentations to local community groups, the College of Pharmacy, other hospitals and pharmacy departments, and professional meetings.
