Scope of Position In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients, families, pharmacists, nurses, physicians, medical students, and other associate hospital personnel. Approves new drug orders, checks and verifies chemotherapy notes, informed consent, and processes non‑formulary requests. Screens for drug‑drug, drug‑herb/dietary supplement, drug‑allergy, drug‑food (in cooperation with the Dietetics Department), drug‑disease state, and drug‑laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to the practice area.
Position Summary This position coordinates all aspects of patients’ pharmaceutical needs in patient care areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug‑drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients’ pharmaceutical needs with physicians and nurses. The patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must demonstrate knowledge and skills necessary to provide age‑appropriate care and medication dosing. The role also supervises Pharm.D. students, technicians, undergrad and graduate student teaching, and provides educational programs for hospital staff (physicians, nurses, and pharmacy staff).
Minimum Qualifications
Registered pharmacist in the State of Ohio; board certified
Advanced degree (Pharm.D./MS) and residency, and/or equivalent experience preferred
Additional Qualifications
Conducts experiential and/or didactic teaching of pharmacy students (including interns), pharmacy residents, and permanent staff
Professional instruction to hospital and clinical staff or other professional organizations
Participates in and presents intra‑departmental continuing education programs and professional presentations to local community groups, the College of Pharmacy, other hospitals, pharmacy departments, and professional meetings
