Position Overview
We are seeking a
dedicated Nurse Practitioner (NP) or Physician Assistant (PA)
to join our Los Angeles team. The ideal candidate is compassionate, detail-oriented, and motivated to make a meaningful difference in the lives of elderly patients. This role combines hands-on patient care with cutting-edge digital health technology, offering an opportunity to practice medicine in a collaborative and forward-thinking environment. Responsibilities
Conduct clinical assessments and provide ongoing care management for patients enrolled in Happy Vitals’ RPM/CCM programs
Review and interpret patient data from monitoring devices, ensuring timely interventions and follow-ups
Collaborate with facility staff, caregivers, and multidisciplinary teams to optimize patient care
Document visits, care plans, and patient progress in EMR systems accurately and promptly
Provide patient and family education on health management, prevention, and wellness strategies
Participate in quality improvement initiatives and clinical team meetings
Qualifications
Current Nurse Practitioner (NP) or Physician Assistant (PA) license in California (required)
Active DEA license (preferred)
Experience in geriatrics, internal medicine, primary care, or long-term care is strongly preferred
Strong communication, organizational, and clinical documentation skills
Comfort working with digital health tools and patient monitoring systems
A passion for improving healthcare delivery through innovation
