The Assertive Community Treatment (ACT) Team serves adults with severe and persistent mental health issues by providing ongoing, intensive services that are essential to assisting and supporting clients as they live in the community, free from psychiatric hospitalization.

Case Manager Responsibilities

Preparing individualized treatment plans

Conducting periodic assessments

Providing skills training and supportive counseling

Advocating for and assisting the consumer with obtaining needed services and maintaining eligibility for social services.

Program Summary

Responsible for the overall management of a program.

Contributes significantly to program development, quality assurance, staff supervision, budgeting and service coordination with other agencies.

Works autonomously in carrying out the goals and functions of the program.

Provides programmatic and consultative assistance to other programs.

Essential Duties and Responsibilities

Develop strategy for long‑range plan to support rapid growth within program.

Provide leadership necessary to facilitate growth and improvement within program.

Work with staff to assist in meeting productivity standards.

Monitor and update policies, procedures and operational guidelines of program to ensure compliance.

Educate agency programs and community resources about program.

Communicate regularly with other Program Directors to stay informed of concerns, questions, needs and achievements.

Be responsible for general oversight of program functionality.

Be responsible for onboarding of clinical staff in conjunction with Human Resources, by scheduling appropriate trainings and shadowing of other clinicians.

Facilitate communication between senior leadership and front‑line staff by attending leadership meetings and serving as a conduit for important information.

Manage the program budget ensuring expenses remain within budget and revenues meet expectations.

Comply with federal, state and company policies, procedures and regulations.

Maintain confidentiality of all information and comply with HIPAA regulations.

Demonstrate knowledge of and support Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.

Qualification Requirements

Must be able to communicate clearly and accurately.

Must have strong decision‑making skills.

Must work well in a team environment.

Must work with staff, consumers and others in a professional, cooperative, ethical, respectful and effective manner.

Must be able to work with a diverse group of people.

Education and/or Experience

Master’s degree and valid master’s level WV license in Counseling, Social Work, Psychology, Supervised Psychologist, or registered nurse.

Three years of experience in behavioral health services, two of which must be in a supervisory capacity.

Prior computer/PC experience using computerized medical records.

Must have a valid driver’s license and pass Criminal Investigation Background check and DHHR background check as well as post‑offer drug screen.

Physical Requirements The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Mobility and ability to sit for long periods of time, use a standard computer keyboard, bend, reach and lift up to 10 pounds are required in this position.
